Join us on August 16th from 10:00 am to 2:00 pm at Nappanee First Brethren Church (1600 N Main St. Nappanee, IN 46550). This Immunization Clinic is by appointment only. For more information or to schedule an appointment, please call (574) 523-2127
Cost: $15 per shot (Fee covered by Insurance). Accepting uninsured, Medicaid, and most other insurances.
Vaccines are given based on guidelines from the Indiana State Department of Health (ISDH) & the Centers for Disease Control and Prevention (CDC).
Please bring the following:
- ALL IMMUNIZATION RECORDS
- INSURANCE INFORMATION
- CHILD MUST BE ACCOMPANIED BY PARENT/GUARDIAN OR AN ADULT (18 YRS. OR OLDER) WITH WRITTEN PERMISSION FROM PARENT
- THE ADULT OR CHILD, IF 13 OR OLDER, WILL BE ASKED TO SHOW A PHOTO ID (YOU MAY SIGN A WAIVER FOR RELIGIOUS OBJECTION)
- GUARDIANS MUST PRESENT LEGAL PAPERWORK
